Please bring an up-to-date list of your current medications.
Referral Letter
Please ensure your referral letter has been sent to our clinic before your appointment. This can be arranged by your GP or sent by you directly.
Arrive 10–15 Minutes Early
If you're a new patient, please arrive 10–15 minutes before your scheduled appointment to complete the necessary paperwork.
